{"id":4075,"date":"2010-10-22T12:58:21","date_gmt":"2010-10-22T16:58:21","guid":{"rendered":"http:\/\/blogs.nejm.org\/cardioexchange\/?p=4075"},"modified":"2011-07-19T17:45:25","modified_gmt":"2011-07-19T21:45:25","slug":"what-do-i-plan-to-do-with-dabigatran","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/cardioexchange\/2010\/10\/22\/what-do-i-plan-to-do-with-dabigatran\/","title":{"rendered":"What Do I Plan to Do with Dabigatran?"},"content":{"rendered":"<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">1.<em> Discuss<\/em><\/span> <\/strong>its pros (no need for INR monitoring, less bleeding than coumadin) and cons (cost, dyspepsia) with my atrial fibrillation patients<\/p>\n<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">2.<em> Avoid<\/em><\/span><\/strong> its use in patients with renal dysfunction (CrCl \u00a0&lt;60), liver disease, pregnancy or stroke within the past 6 months<\/p>\n<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">3.<em> Lament<\/em><\/span> <\/strong>the absence of the INR monitoring requirement, which I currently use to (a) check on patient compliance with medication; (b) assess the adequacy of anticoagulation (especially in patients near the extremes of weight); and (c) detect potential drug interactions (although purportedly rare)<\/p>\n<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">4.<em> Worry<\/em><\/span><\/strong> that (a) the patient given dabigatran because of probable noncompliance with INR monitoring won\u2019t comply with a twice daily drug regimen; and (b) I can\u2019t reverse dabigatran\u2019s effects when the patient has a major bleeding episode<\/p>\n<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">5.<em> Wait<\/em><\/span><\/strong> to see (a) whether future studies show a higher risk of MI with dabigatran than coumadin (as noted in RE-LY) and (b) if dabigatran is as effective as coumadin in patients with mechanical valves<\/p>\n<p style=\"margin-left: .5in; text-indent: -.25in;\"><strong><span style=\"color: #000000;\">6.<em> Hide<\/em> <\/span><\/strong>when the patient (or insurance company) gets the bill for the medication (estimated cost: ~$7,000 to $9,000 per patient-year, which is 4 to 5 times the cost of coumadin, even taking into account the increased physician and laboratory costs required to monitor INR).<\/p>\n<p><em>Comments are closed on this post, but please join the conversation at our <a href=\"http:\/\/blogs.nejm.org\/cardioexchange\/fda-approves-dabigatran-for-stroke-prevention-in-atrial-fibrillation\/dabigatran-resource-round-up\/\">Dabigatran Resource Round-Up<\/a>.<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>1.
