The novel drug from\u00a0Novartis\u00a0was intended to be used in patients with acute heart failure (HF).\u00a0The once highly promising drug, which\u00a0<a title=\"Novel Heart Failure Drug from Novartis Gains \u2018Breakthrough Therapy\u2019 Designation from FDA\" href=\"http:\/\/blogs.nejm.org\/cardioexchange\/news\/novel-heart-failure-drug-from-novartis-gains-breakthrough-therapy-designation-from-fda\/\">received a\u00a0\u201dbreakthrough therapy\u201d\u00a0designation<\/a>\u00a0from the FDA last year, was also\u00a0<a title=\"European Setback for Novel Heart Failure Drug\" href=\"http:\/\/blogs.nejm.org\/cardioexchange\/news\/european-setback-for-novel-heart-failure-drug\/\">turned down for approval in\u00a0Europe<\/a>\u00a0earlier this year.<\/p>\n<p>FDA reviewers and committee members expressed no significant concerns over the safety of the drug. But they were troubled by the seemingly impenetrable results of\u00a0<a title=\"RELAX-AHF Stirs Interest in Novel Drug for Acute Heart Failure\" href=\"http:\/\/blogs.nejm.org\/cardioexchange\/news\/relax-ahf-stirs-interest-in-novel-drug-for-acute-heart-failure\/\">the pivotal RELAX-AHF trial<\/a>. Throughout the day the Novartis speakers, including HF specialists Milton Packer\u00a0and RELAX-AHF investigator Barry Greenberg, tried to explain and justify the complex trial, which only met one of its two co-primary endpoints.<\/p>\n<p>But panel members remained skeptical. They found that flaws in trial design made it impossible to accurately assess the effect of the drug.\u00a0They did not completely reject\u00a0<a title=\"Novartis Announces Top Line Results for Phase 3 Trial of New Acute Heart Failure Drug\" href=\"http:\/\/blogs.nejm.org\/cardioexchange\/news\/novartis-announces-top-line-results-for-phase-3-trial-of-new-acute-heart-failure-drug\/\">the surprising finding of a mortality reduction<\/a>\u00a0at 180 days in RELAX-AHF, but because it was not a prespecified endpoint they said it needed to be tested in a followup study.<\/p>\n<p>After the vote most of the panelists advised the company to continue development of the drug since it may ultimately help address an important unmet medical need.<\/p>\n<p>Novartis sent the following statement after the panel meeting:<\/p>\n<blockquote>\n<p style=\"text-align: left;\">&#8220;Recognizing the urgent patient need, today we presented what we believe to be a persuasive picture of the evidence for RLX030 so far \u2013 compelling results from our Phase II and III trials with no significant safety concerns,&#8221; said Tim Wright, Global Head of Development, Novartis Pharmaceuticals. &#8220;The discussion provided important information that we will address with the FDA as it completes its review.
