{"id":45359,"date":"2014-09-23T14:14:41","date_gmt":"2014-09-23T18:14:41","guid":{"rendered":"http:\/\/blogs.nejm.org\/cardioexchange\/?post_type=news&#038;p=45359"},"modified":"2014-09-23T14:14:41","modified_gmt":"2014-09-23T18:14:41","slug":"acc-and-aha-release-new-guideline-for-non-st-elevation-acute-coronary-syndromes","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/cardioexchange\/2014\/09\/23\/acc-and-aha-release-new-guideline-for-non-st-elevation-acute-coronary-syndromes\/","title":{"rendered":"ACC and AHA Release New Guideline for Non\u2013ST-Elevation Acute Coronary Syndromes"},"content":{"rendered":"<p>A <a href=\"http:\/\/circ.ahajournals.org\/content\/early\/2014\/09\/22\/CIR.0000000000000134\">new guideline<\/a> released today by the American College of Cardiology and the American Heart Association reflects important changes in the last 7 years in the understanding of what is now known as non\u2013ST-elevation acute coronary syndromes (NSTE-ACS).<\/p>\n<p>The previous 2007 guideline used different terminology and referred to the same patient population as &#8220;unstable angina and non-ST-elevation myocardial infarction (NSTEMI)&#8221;.\u00a0The new terminology &#8220;emphasizes the pathophysiologic continuum of unstable angina and NSTEMI and their frequently indistinguishable clinical presentations,&#8221; according to a <a href=\"http:\/\/newsroom.heart.org\/news\/guideline-aims-to-improve-outcomes-of-patients-with-nste-acs\">joint press release<\/a> from the AHA and ACC.<\/p>\n<p>Although NSTE-ACS patients with significant coronary disease are generally eligible for an early invasive strategy, some low-risk patients may be eligible for medical therapy.\u00a0\u201cGuideline-directed medical therapy has not always been optimally utilized and advances in noninvasive testing have the potential to identify patients with NSTE-ACS at low-intermediate risk to distinguish candidates for invasive versus medical therapy,\u201d says writing committee chair Ezra A. Amsterdam.<\/p>\n<p>The new guideline also drops the term \u00a0\u201cinitial conservative management\u201d in favor of \u201cischemia-guided strategy\u201d in order &#8220;to more clearly convey the physiologic rationale of this approach.&#8221;<\/p>\n<p>One major focus is the long-term treatment of patients following the acute phase, said the\u00a0vice chair of the writing committee, Nanette Wenger.\u00a0&#8220;The hospitalization period involves crisis management of ACS, which is pivotal to successful patient outcomes during the acute phase of disease.
