A challenging 1586-meter summit that offered magnificent views of Lake Wanaka and snow-capped mountains of the Southern Alps. On this hike, devoid of phone calls and pages, I reflected on my journey as an aspiring clinician-educator. Many of us (like my fellow blogger Prarthna in <a href=\"https:\/\/blogs.nejm.org\/general-medicine\/index.php\/2019\/12\/why-did-i-spend-an-extra-year-as-a-chief-resident\/\" target=\"_blank\" rel=\"noopener noreferrer\">Why Did I Spend an Extra Year as a Chief Resident?<\/a>) want to teach, become better at this skill, and have a career that supports this goal.<\/p>\n<h2><strong>So, how do we create careers as clinician-educators (with a big E)?<\/strong><\/h2>\n<div id=\"attachment_2751\" style=\"width: 310px\" class=\"wp-caption alignleft\"><a href=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/IMG_4736.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-2751\" class=\"size-medium wp-image-2751\" src=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/IMG_4736-300x225.jpg\" alt=\"\" width=\"300\" height=\"225\" srcset=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/IMG_4736-300x225.jpg 300w, https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/IMG_4736-768x576.jpg 768w, https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/IMG_4736-1024x768.jpg 1024w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><p id=\"caption-attachment-2751\" class=\"wp-caption-text\">Feeling exhilarated to be at the peak!\u00a0<a href=\"https:\/\/www.doc.govt.nz\/parks-and-recreation\/places-to-go\/otago\/places\/wanaka-area\/things-to-do\/roys-peak-track\">Roys Peak in South Island, New Zealand<\/a><\/p><\/div>\n<p>In medicine, the clinician-educator term refers to, \u201ca physician whose primary role is caring for patients and who has formally incorporated educational principles and scholarship into her\/ his job description.\u201d These physicians distinguish themselves from other clinicians by, \u201cusing evidence-based educational constructs to teach around patient illness and wellness, and converting those constructions and innovations into scholarship.\u201d <a href=\"https:\/\/journals.lww.com\/academicmedicine\/Fulltext\/2018\/12000\/The_Evolution_of_the_Clinician_Educator_in_the.15.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">(Acad Med 2018; 98: 1764)<\/a><\/p>\n<p><a href=\"https:\/\/www.harvardmacy.org\/index.php\/hmi-courses\/pgme\" target=\"_blank\" rel=\"noopener noreferrer\">Harvard Macy Institute&#8217;s program for postgraduate trainees<\/a> is one way to gain theoretical and practical skills towards a clinician-educator career. I had the privilege of attending last month. This three-day intensive program is paired with a year-long scholarly project in medical education and serves as an entry into the Harvard Macy community. I walked away from the course feeling excited and inspired. I\u2019d like to share some highlights of what I learned from the course and about myself.<\/p>\n<h2><\/h2>\n<h2><\/h2>\n<h2><strong>What I learned from the course<\/strong><\/h2>\n<div id=\"attachment_2753\" style=\"width: 279px\" class=\"wp-caption alignright\"><a href=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM1.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-2753\" class=\"size-medium wp-image-2753\" src=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM1-269x300.png\" alt=\"\" width=\"269\" height=\"300\" srcset=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM1-269x300.png 269w, https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM1.png 334w\" sizes=\"auto, (max-width: 269px) 100vw, 269px\" \/><\/a><p id=\"caption-attachment-2753\" class=\"wp-caption-text\">Kirkpatrick&#8217;s Outcomes Hierarchy<\/p><\/div>\n<ul>\n<li><strong>How to turn innovative ideas into scholarship<\/strong><\/li>\n<\/ul>\n<p>Beckman and Cook (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17701635\" target=\"_blank\" rel=\"noopener noreferrer\"><span role=\"menubar\">Med Teach<\/span> 2007; 29:210<\/a>) is a must-read for anyone interested in medical education! These authors describe a three-step approach to designing scholarly education projects grounded in the theoretical basis of Boyer and Glassick: 1. Refine the study question, 2. Identify design and methods, and 3. Outcomes. In particular, Kirkpatrick\u2019s outcomes hierarchy has been invaluable in developing targeted outcomes.<\/p>\n<ul>\n<li><strong>Practical tips for advancing my scholarly project<\/strong><\/li>\n<\/ul>\n<p>I am forever grateful to my project group with mentors\u00a0<a href=\"https:\/\/twitter.com\/aleichtner\" target=\"_blank\" rel=\"noopener noreferrer\">Alan Leichtner<\/a> and\u00a0<a href=\"https:\/\/www.dana-farber.org\/find-a-doctor\/arielle-l-langer\/\" target=\"_blank\" rel=\"noopener noreferrer\">Arielle Langer<\/a>, and members\u00a0<a href=\"https:\/\/twitter.com\/YakiraDavid\" target=\"_blank\" rel=\"noopener noreferrer\">Yakira<\/a>, Christina, and Larissa. Not only were we a powerhouse team of #WomeninMedicine (and Alan), but we shared thoughtful feedback on each of our projects and practical tips for success. Arielle would often emphasize celebrating the small wins; an embrace of the small advances of our work.<\/p>\n<ul>\n<li><strong>Ways to seek\u00a0training in medical education<\/strong><\/li>\n<\/ul>\n<p>In addition to training at the Harvard Macy Institute, we discussed ways to seek out educator development opportunities at our own institutions (for example, a medical education residency track). Coursework is also available at the\u00a0<a href=\"http:\/\/www.preceptor.ca\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">Preceptor Education Program for Health Professionals and Students<\/a>. And lastly, fellowship training in medical education also exists at Stanford and\u00a0<a href=\"https:\/\/www.bidmc.org\/medical-education\/rabkin-fellowship\" target=\"_blank\" rel=\"noopener noreferrer\">Rabkin\/Beth Israel Deaconess Medical Center.<\/a><\/p>\n<h2><strong>What I learned about myself<\/strong><\/h2>\n<div id=\"attachment_2758\" style=\"width: 310px\" class=\"wp-caption alignright\"><a href=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM2.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-2758\" class=\"size-medium wp-image-2758\" src=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM2-300x234.png\" alt=\"\" width=\"300\" height=\"234\" srcset=\"https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM2-300x234.png 300w, https:\/\/blogs.nejm.org\/general-medicine\/wp-content\/uploads\/sites\/4\/2019\/12\/NEJM2.png 574w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><p id=\"caption-attachment-2758\" class=\"wp-caption-text\">2019 David A. Kolb, Experience Based Learning Systems, Inc.<\/p><\/div>\n<ul>\n<li><strong>How I learn and teach<\/strong><\/li>\n<\/ul>\n<p>I learned that I have an accommodating learning style (a combination of concrete experience and active experimentation; Kolb learning style inventory) and an apprenticeship perspective on teaching (<a href=\"http:\/\/www.teachingperspectives.com\/\" target=\"_blank\" rel=\"noopener noreferrer\">Teaching Perspectives Inventory<\/a>).<\/p>\n<ul>\n<li><strong>The joy of micro-teaching<\/strong><\/li>\n<\/ul>\n<p>I taught a micro-teaching session on \u2018how to run a marathon\u2019 with my group members pretending to be runners. It was an energizing exercise that included the teaching segment being recorded and then viewed by me for self-reflection and by the group for feedback. Sometimes the journey toward becoming a clinician-educator can feel like scaling a 1586-meter summit. Currently, she is honing her skills as a clinician educator through the Harvard Macy Institute and evaluating missed opportunities for pre-exposure prophylaxis (PrEP) at CHA. Her favorite pastimes include trekking through the alps on the Tour du Mont Blanc and trail running in Cape Town. She recently ran the 2019 NYC marathon. A serendipitous decision to pursue a health and literacy internship near Varanasi, India, became the starting point for an incredibly meaningful 9 years, working in the field of international health as a community organizer, public health researcher, and now, as a medical professional. She received a Master\u2019s of Public Health from Columbia University and has worked across sub-Saharan Africa and North America. Through her travels, she has found joy in learning with and from diverse, underserved communities and has sought opportunities to advance social justice. She attended Saint Louis University School of Medicine and trained in Internal Medicine at Cambridge Health Alliance (CHA) in Massachusetts, where she is now Chief Resident. Currently, she is honing her skills as a clinician educator through the Harvard Macy Institute and evaluating missed opportunities for pre-exposure prophylaxis (PrEP) at CHA. Her favorite pastimes include trekking through the alps on the Tour du Mont Blanc and trail running in Cape Town. She recently ran the 2019 NYC marathon.
