{"id":103,"date":"2008-10-21T18:50:43","date_gmt":"2008-10-21T23:50:43","guid":{"rendered":"http:\/\/blogs.nejm.org\/pov\/hiv-id-observations\/?p=103"},"modified":"2008-10-21T18:50:43","modified_gmt":"2008-10-21T23:50:43","slug":"back-to-school-day-4-pep-and-more-pep","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/back-to-school-day-4-pep-and-more-pep\/2008\/10\/21\/","title":{"rendered":"Back to School, Day 4: PEP and More PEP"},"content":{"rendered":"<p>After a lecture on <em>HIV for Primary Care Providers <\/em>in our course last week, the most controversial topic was, not surprisingly, the use of post-exposure prophylaxis (PEP) for both occupational and non-occupational exposures.\u00a0 And today, after an entire lecture on PEP to a group of HIV providers in our AIDS course, again the subject drew numerous questions &#8212; and strong opinions &#8212; from the audience.<\/p>\n<p>Since this is a relatively data-free zone, one <a href=\"http:\/\/www.aidsinfo.nih.gov\/Guidelines\/GuidelineDetail.aspx?MenuItem=Guidelines&amp;Search=Off&amp;GuidelineID=10&amp;ClassID=3\" target=\"_blank\">turns to the guidelines<\/a> for advice.\u00a0 But not surprisingly, they offer tons of wiggle room for a clinician to do pretty much anything he or she wants in all but the most florid exposures or non-exposures.<\/p>\n<p>(Can there be a florid non-exposure?)<\/p>\n<p>So <a href=\"http:\/\/aids-clinical-care.jwatch.org\/cgi\/content\/full\/2008\/1020\/1?q=featured_ac\" target=\"_blank\">here&#8217;s a case we just posted on <em>AIDS Clinical Care.<\/em><\/a> (Drawn from real life, of course.)\u00a0 Emergency room resident sticks herself with a needle while suturing a patient&#8217;s wound, a patient who&#8217;s HIV positive with an undetectable viral load on treatment.
