{"id":1038,"date":"2010-07-30T10:39:40","date_gmt":"2010-07-30T14:39:40","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=1038"},"modified":"2015-06-04T15:24:52","modified_gmt":"2015-06-04T19:24:52","slug":"perinatal-transmission-of-hiv-solved-now-how-do-we-pay-for-it","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/perinatal-transmission-of-hiv-solved-now-how-do-we-pay-for-it\/2010\/07\/30\/","title":{"rendered":"Perinatal Transmission of HIV &#8220;Solved&#8221; &#8212; Now How Do We Pay For It?"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-full wp-image-1039\" title=\"mother and baby\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2010\/07\/mother-and-baby1.jpg\" alt=\"mother and baby\" width=\"227\" height=\"337\" \/>Conspicuously absent from this year&#8217;s International AIDS Conference were major studies on prevention of maternal-to-child transmission.<\/p>\n<p>It could be that I just missed them, so I emailed a colleague who specializes in the area, and she concurred:<\/p>\n<blockquote><p>Nope, did not see or hear major PMTCT updates at IAS.<\/p><\/blockquote>\n<p>The thing is, this problem has been all but solved, at least scientifically.\u00a0 Put the mom on <a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa0907736\" target=\"_blank\">fully suppressive treatment<\/a>, and the rest of what\u00a0is done\u00a0barely matters &#8212; mode of delivery, HIV treatment to the baby, breast vs formula feeding.\u00a0 It&#8217;s all a wash, because HIV transmission will barely ever occur.<\/p>\n<p>(Yes, there are numerous other questions <em>unrelated<\/em> to HIV transmission, most notably the safest regimens from the perspective of\u00a0in-utero exposure.\u00a0 Bold prediction:\u00a0 one day we will no longer be using zidovudine.)<\/p>\n<p>So &#8212; now\u00a0that we know what to do (treat the moms),\u00a0we just have to learn how to pay for it.\u00a0 Easier said than done.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Conspicuously absent from this year&#8217;s International AIDS Conference were major studies on prevention of maternal-to-child transmission.
