{"id":1108,"date":"2010-10-01T17:26:01","date_gmt":"2010-10-01T21:26:01","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=1108"},"modified":"2015-06-04T15:24:36","modified_gmt":"2015-06-04T19:24:36","slug":"five-friday-fasciolas","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/five-friday-fasciolas\/2010\/10\/01\/","title":{"rendered":"Five Friday Fasciolas"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-full wp-image-1109\" title=\"watercress_salad\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2010\/10\/watercress_salad1.jpg\" alt=\"watercress_salad\" width=\"300\" height=\"289\" \/>As we await either the start of the baseball playoffs (or <a href=\"http:\/\/www.soxaholix.com\/tp\/2010\/09\/other-echoes.html\" target=\"_blank\">Spring Training<\/a>), here is some ID\/HIV content to consider, in no particular order:<\/p>\n<ol>\n<li>Could adenovirus infection be the cause of obesity?\u00a0 That would be the media take, especially from this highly-esteemed research journal, <a href=\"http:\/\/www.nydailynews.com\/lifestyle\/2010\/09\/22\/2010-09-22_obesity_in_children_could_be_due_to_a_cold_virus_study.html\" target=\"_blank\">The New York Daily News<\/a>. (Warning:\u00a0 kind of ugly photo in link.)\u00a0 But if you read <a href=\"http:\/\/pediatrics.aappublications.org\/cgi\/content\/abstract\/peds.2009-3362v1\" target=\"_blank\">the actual paper<\/a>, the scientists appropriately\u00a0note that the study just demonstrated an association with childhood obesity, not causation.\u00a0 Still &#8212; did you know that animal models show that adenovirus 36 infection increases body fat?\u00a0 I did not.<\/li>\n<li>Remember ceftobiprole?\u00a0 The &#8220;Fifth Generation&#8221; anti-MRSA cephalosporin approved for use in Europe, Canada, Hong Kong, etc and heading to the USA?\u00a0Well you can take <a href=\"http:\/\/www.pharmatimes.com\/Article\/10-04-12\/Sales_of_Basilea_s_antibiotic_Zeftera_stopped_in_Canada.aspx\" target=\"_blank\">Canada<\/a>,\u00a0<a href=\"http:\/\/www.reuters.com\/article\/idUS42020+09-Sep-2010+HUG20100909\" target=\"_blank\">Switzerland<\/a> and soon several other countries (Russia, Ukraine and Azerbaijan) off that list, as the drug is being pulled due to <a href=\"http:\/\/blogs.nejm.org\/index.php\/the-long-road-for-ceftobiprole-approval-gets-longer\/2010\/01\/10\/\" target=\"_blank\">concerns raised by the FDA<\/a> in their review.\u00a0 By contrast, it does look\u00a0like ceftaroline (reminds me of <a href=\"http:\/\/www.google.com\/images?q=caroline+kennedy&amp;um=1&amp;ie=UTF-8&amp;source=og&amp;sa=N&amp;hl=en&amp;tab=wi&amp;biw=1283&amp;bih=847\" target=\"_blank\">Caroline<\/a>,\u00a0less so\u00a0<a href=\"http:\/\/www.hydeparkpedi.com\/about_sax.html\" target=\"_blank\">Carolyn<\/a>) is on the way. My reason for optimism this time is the <a href=\"http:\/\/www.frx.com\/news\/PressRelease.aspx?ID=1468014\" target=\"_blank\">uninimously favorable\u00a0vote<\/a> from\u00a0an FDA Advisory Committee meeting; <a href=\"http:\/\/www.fda.gov\/AdvisoryCommittees\/CommitteesMeetingMaterials\/Drugs\/Anti-InfectiveDrugsAdvisoryCommittee\/ucm225752.htm\" target=\"_blank\">here are the slides<\/a> from the meeting last month.<\/li>\n<li>This <a href=\"http:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm5937a2.htm?s_cid=mm5937a2_w\" target=\"_blank\">MMWR paper<\/a> on the prevalence of HIV among gay men in the USA got lots of attention, and appropriately so.\u00a0 The reason?\u00a0 &#8220;<em>New CDC analysis finds 1 in 5 gay and bisexual men in a study of 21 major U.S. cities is infected with HIV, and nearly half (44 percent) are unaware; young men and men of color are least likely to know their status.&#8221; <\/em>Important take-home message:\u00a0 In the effort to\u00a0expand general HIV screening strategies, we can&#8217;t forget that there&#8217;s a critical role for targeted testing as well.\u00a0\u00a0 Some would argue this is <em>more<\/em> important.<\/li>\n<li>One of the most widely-cited papers in all of HIV medicine in the late 1990s\/early 2000s\u00a0was <a href=\"http:\/\/annals.org\/content\/133\/1\/21.1.full\">this one<\/a>, which showed that you needed to take 95% of HIV medications\u00a0for the best shot at \u00a0suppressing the virus.\u00a0 (I must have some fifty copies of this study in various slide sets scattered around my hard drive.)\u00a0 We&#8217;ve known for some time that the adherence bar isn&#8217;t set this high\u00a0with today&#8217;s better regimens, but\u00a0could one ever imagine that\u00a0someone could take <a href=\"http:\/\/journals.lww.com\/jaids\/Abstract\/publishahead\/Risk_of_Viral_Failure_Declines_With_Duration_of.98912.aspx\" target=\"_blank\">as little as\u00a040%<\/a> of their meds without much risk of viral rebound, provided it&#8217;s been preceded by long term virologic suppression?\u00a0\u00a0 Not that I&#8217;m suggesting this as an actual strategy, but this should be quite reassuring for our successfully treated patients who think they <em>can never miss a dose.<\/em><\/li>\n<li>Easily one of the <a href=\"http:\/\/www.woodtv.com\/dpp\/news\/local\/kent_county\/health-department-informed-of-impersonator-providing-HIV-information\" target=\"_blank\">un-funniest pranks<\/a> I&#8217;ve heard about, maybe ever.
