{"id":1880,"date":"2011-08-31T09:23:16","date_gmt":"2011-08-31T13:23:16","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=1880"},"modified":"2015-06-04T15:15:51","modified_gmt":"2015-06-04T19:15:51","slug":"its-time-for-antibiotic-placebos","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/its-time-for-antibiotic-placebos\/2011\/08\/31\/","title":{"rendered":"It&#8217;s Time for Antibiotic Placebos"},"content":{"rendered":"<p><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2011\/08\/azithro-for-bronchitis1.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-full wp-image-1891\" title=\"azithro for bronchitis\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2011\/08\/azithro-for-bronchitis1.jpg\" alt=\"\" width=\"202\" height=\"194\" \/><\/a>As I&#8217;m sure you all agree, it&#8217;s high time we had a good antibiotic placebo.<\/p>\n<p>Just think &#8212; we&#8217;d be able to prescribe a 100% effective treatment for viral respiratory tract infections, with the assurance of no risk of antibiotic resistance, C diff, allergic reactions, tendon ruptures, photosensitivity, drug-drug interactions, or any of the myriad other side effects that real antibiotics have.<\/p>\n<p>Plus, based on <a href=\"http:\/\/www.plosone.org\/article\/info:doi\/10.1371\/journal.pone.0015591\" target=\"_blank\">studies like this one<\/a>, we could even tell our patients that they&#8217;re receiving an antibiotic placebo, and they&#8217;d still get better. There, ethical quandaries of placebo prescribing solved.<\/p>\n<p>What inspired this thought on antibiotic placebos was this email exchange:<\/p>\n<blockquote><p>Hi Paul,<br \/>\nOur primary care group would love to have you come give an update on antibiotics during our Friday conference. Am cc&#8217;ing Chuck our practice director to make sure he agrees with the topic. There&#8217;s a lot of variability in antibiotic prescribing within our group, hoping you can enlighten us.<br \/>\nThanks,<br \/>\nNora<\/p><\/blockquote>\n<p>So Chuck chimed in:<\/p>\n<blockquote><p>Sounds great. \u00a0But Nora, what do you mean &#8220;variability in antibiotic prescribing&#8221; if everyone gets azithro?<br \/>\nChuck<\/p><\/blockquote>\n<p>Now it hardly bears mentioning that Chuck is one of the funniest doctors I know, and that he was being facetious.<\/p>\n<p>But just think how much macrolide resistance we could avoid if, instead of a &#8220;Z-pack&#8221;, we had an &#8220;S-pack&#8221; (&#8220;S&#8221; for sucrose). That&#8217;s 6 pills (two on the first day, then one a day for 4 additional days), with the promise that it will be just as effective as azithromycin.<\/p>\n<p>And if you don&#8217;t believe me, read this paper in <em><a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(02)08597-5\/abstract\" target=\"_blank\">The Lancet<\/a><\/em> for proof (figure from paper in above image).
