{"id":2269,"date":"2011-12-28T08:23:09","date_gmt":"2011-12-28T13:23:09","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=2269"},"modified":"2015-06-04T15:15:14","modified_gmt":"2015-06-04T19:15:14","slug":"why-we-still-need-hivid-specialists","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/why-we-still-need-hivid-specialists\/2011\/12\/28\/","title":{"rendered":"Why We Still Need HIV\/ID Specialists"},"content":{"rendered":"<p><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2011\/12\/9596678_blog1.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-full wp-image-2270\" title=\"what next\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2011\/12\/9596678_blog1.jpg\" alt=\"\" width=\"280\" height=\"210\" \/><\/a>Over on <em>Journal Watch AIDS Clinical Care<\/em>, we periodically publish a tricky case &#8212; always drawn from clinical practice &#8212; then ask some experts how they would manage it, and why.<\/p>\n<p>The <a href=\"http:\/\/aids-clinical-care.jwatch.org\/cgi\/content\/full\/2011\/1222\/1?q=featured_ac\" target=\"_blank\">most recent case <\/a>pretty much has it all:<\/p>\n<ul>\n<li>Multiple prior regimens<\/li>\n<li>Multi-class drug resistance<\/li>\n<li>Metabolic complications<\/li>\n<li>Bad allergy history, one event nearly requiring hospitalization<\/li>\n<li>Disfiguring lipoatrophy<\/li>\n<li>History of treatment discontinuation<\/li>\n<\/ul>\n<p>In fact, the case is so complicated that our Executive Editor said it made her &#8220;head hurt.&#8221; She&#8217;s just lucky the guy doesn&#8217;t have hepatitis C.<\/p>\n<p>Fortunately, <a href=\"http:\/\/www.hivvirology.com\/editors\" target=\"_blank\">Graeme Moyle<\/a> and <a href=\"http:\/\/www.uphs.upenn.edu\/idd\/mounzer.html\" target=\"_blank\">Karam Mounzer<\/a> kindly agreed to take it on.
