{"id":262,"date":"2009-01-29T10:41:06","date_gmt":"2009-01-29T15:41:06","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=262"},"modified":"2015-06-04T15:31:19","modified_gmt":"2015-06-04T19:31:19","slug":"too-many-options-what-actually-happened","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/too-many-options-what-actually-happened\/2009\/01\/29\/","title":{"rendered":"Too Many Options:  What Actually Happened"},"content":{"rendered":"<p><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2009\/01\/no_nukes1.gif\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-thumbnail wp-image-264\" title=\"no_nukes\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2009\/01\/no_nukes1.gif\" alt=\"\" width=\"120\" height=\"120\" \/><\/a><\/p>\n<p>We recently <a href=\"http:\/\/aids-clinical-care.jwatch.org\/cgi\/content\/full\/2009\/112\/1\" target=\"_blank\">published a case<\/a> in <em>AIDS Clinical Care <\/em>entitled &#8220;Too Many Options&#8221;, describing a patient with longstanding HIV infection, virologic failure, and resistance to NRTIs, NNRTIs, and PIs.<\/p>\n<p>Fortunately, resistance and tropism testing gave him several options for a new drug regimen &#8212; including darunavir, etravirine, maraviroc, enfuvirtide, and &#8212; if one believes phenotypic NRTI susceptibility with multiple TAMs &#8212; several NRTIs.<\/p>\n<p>And what did our three experts suggest?\u00a0 Three <em>different <\/em>regimens:<\/p>\n<ul>\n<li><a href=\"http:\/\/www.uhnres.utoronto.ca\/researchers\/profile.php?lookup=6344\" target=\"_blank\">Sharon Walmsley<\/a>:\u00a0 darunavir\/r, maraviroc, and etravirine (three drugs &#8212; not counting ritonavir)<\/li>\n<\/ul>\n<ul>\n<li><a href=\"http:\/\/www.weillcornell.org\/twilkin\/\" target=\"_blank\">Tim Wilkin<\/a>:\u00a0 darunavir\/r, raltegravir, maraviroc, and tenofovir\/FTC (five drugs)<\/li>\n<\/ul>\n<ul>\n<li><a href=\"http:\/\/www.thebody.com\/content\/art39635.html\" target=\"_blank\">Graeme Moyle<\/a>: darunavir\/r, raltegravir, and maraviroc (three drugs, but different from Sharon&#8217;s selection)<\/li>\n<\/ul>\n<p>Now these are smart, highly-experienced clinicians, physicians who are active in clinical research, know the literature extremely well, and actually see patients.\u00a0 (Funny how that last part is sometimes left out.) \u00a0 Each of them provided sound reasons for their (varying) choices.<\/p>\n<p>And what did we do with this patient?\u00a0 (Or one very much like him &#8230; obviously some details changed as per <a href=\"http:\/\/www.hhs.gov\/ocr\/privacy\/index.html\" target=\"_blank\">HIPAA<\/a> mandate.)<\/p>\n<p>We offered him the chance to enroll in the clinical study ACTG 5241 (mentioned by Tim), which takes patients like this, gives them an optimal regimen &#8212; then randomizes them to receive or not to receive NRTIs.<\/p>\n<p>(I don&#8217;t think it will ruin the study to mention that the &#8220;flip of the coin&#8221; gave him the &#8220;No Nukes&#8221; option.)<\/p>\n<p>I sure hope we learn something from this study.\u00a0 While we know that regimens should contain &#8220;two (preferably three)&#8221; active agents, beyond that there&#8217;s plenty of uncertainty out there.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>We recently published a case in AIDS Clinical Care entitled &#8220;Too Many Options&#8221;, describing a patient with longstanding HIV infection, virologic failure, and resistance to NRTIs, NNRTIs, and PIs.
