{"id":28,"date":"2008-07-11T20:25:48","date_gmt":"2008-07-12T01:25:48","guid":{"rendered":"http:\/\/blogs.nejm.org\/pov\/hiv-id-observations\/index.php\/2008\/07\/11\/m184v-so-many-options-but-does-that-include-tdfftcefv\/"},"modified":"2008-07-11T20:25:48","modified_gmt":"2008-07-12T01:25:48","slug":"m184v-so-many-options-but-does-that-include-tdfftcefv","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/m184v-so-many-options-but-does-that-include-tdfftcefv\/2008\/07\/11\/","title":{"rendered":"M184V:  So many options, but does that include TDF\/FTC\/EFV?"},"content":{"rendered":"<p>Co-formulated TDF\/FTC\/EFV (Atripla)\u00a0is a nifty bit of pharmacologic packaging (ever so much more so since it involves collaboration between two different pharmaceutical companies, ahem) &#8212;\u00a0and our patients have noticed.\u00a0 All of us who practice HIV medicine have been asked for the &#8220;one pill&#8221; treatment; often these requests make sense, sometimes they don&#8217;t.<\/p>\n<p>It&#8217;s easy to say when it&#8217;s a bad idea (known NNRTI resistance, for example), but\u00a0sometimes it&#8217;s not so clear.<\/p>\n<p><a href=\"http:\/\/aids-clinical-care.jwatch.org\/cgi\/content\/full\/2008\/609\/2\">We presented a case in <em>AIDS Clinical Care<\/em><\/a> of someone with documented M184V several years previously (before a treatment interruption) who now needed to go back on treatment.\u00a0 Her request:\u00a0 the &#8220;one pill&#8221; treatment.\u00a0 One of our clinical experts (Joel Gallant) said he wouldn&#8217;t do it; the other (Jose Arribas) said he would.\u00a0 Not surprisingly, there were good reasons provided by both for their decisions.<\/p>\n<p>We also asked if they&#8217;d order a viral tropism test.\u00a0 Joel &#8212; no.\u00a0 Jose &#8212; yes.<\/p>\n<p>Thoughts?\u00a0 What do you do for patients who only have M184V?<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Co-formulated TDF\/FTC\/EFV (Atripla)\u00a0is a nifty bit of pharmacologic packaging (ever so much more so since it involves collaboration between two different pharmaceutical companies, ahem) &#8212;\u00a0and our patients have noticed.\u00a0 All of us who practice HIV medicine have been asked for the &#8220;one pill&#8221; treatment; often these requests make sense, sometimes they don&#8217;t.
