Terrified? I bet your department is buzzing about this.<\/p><\/blockquote>\n<p>Um, not <em>quite<\/em> &#8212; especially since, among the 49 cases in the world (apparently there are 5 more than the WHO reported), exactly zero have occurred thus far in the United States. As of May 29, 2013, it hasn&#8217;t even cracked the front page of <a href=\"http:\/\/www.cdc.gov\/\" target=\"_blank\">the CDC site.<\/a><\/p>\n<p>Is MERS-CoV potentially of great concern? Of course. The <a href=\"http:\/\/www.who.int\/csr\/don\/2013_05_23_ncov\/en\/index.html\" target=\"_blank\">WHO response<\/a> seems right, especially with the parallels to SARS.<\/p>\n<p>But do we garden-variety ID specialists know how serious it will be on a global basis? Of course not. As with the first SARS cases, the first anthrax cases, the first West Nile cases, the first hantavirus cases, even the first AIDS cases &#8212; we really don&#8217;t have enough points on the graph yet to make any sort of confident predictions.<\/p>\n<p>And from a practical perspective, the clinical unfamiliarity doesn&#8217;t help. If someone walked into our emergency room tomorrow with fever, cough, and respiratory symptoms, would we know how to distinguish\u00a0MERS-CoV from the hundreds (OK, thousands) of other causes of similar illnesses?<\/p>\n<p>Initially, not a chance. The denominator of people with these complaints is just too gargantuan. It will probably take someone with a particularly severe respiratory illness, along with the appropriate exposure (&#8220;He just returned from a 10-day business trip to Riyadh&#8221;) for an astute clinician to make the connection.<\/p>\n<p>So how should we ID doctors, who are supposed to know everything, respond to these emails in the interim? Stay informed efficiently by consulting these sites:<\/p>\n<ol>\n<li><a href=\"http:\/\/www.promedmail.org\/\" target=\"_blank\">ProMed-mail<\/a>\u00a0(outbreak updates from your ID colleagues\/friends around the world).<\/li>\n<li>The always-fascinating <a href=\"http:\/\/www.cdc.gov\/outbreaks\/index.html\" target=\"_blank\">CDC Outbreaks page<\/a>, plus it&#8217;s <a href=\"http:\/\/www.cdc.gov\/Features\/novelcoronavirus\/\" target=\"_blank\">coronavirus-specific info.<\/a><\/li>\n<li>Some non-MD news source &#8212; I like\u00a0<a href=\"https:\/\/www.google.com\/search?hl=en&amp;gl=us&amp;tbm=nws&amp;q=mers+coronovirus&amp;oq=mers+coronovirus&amp;gs_l=news-cc.3..43j43i53.5530.23853.0.23941.39.21.2.14.16.7.204.1847.13j6j1.20.0...0.0...1ac.1j2.SRTBRv8eg1Y#hl=en&amp;gl=us&amp;tbm=nws&amp;q=mers+coronavirus&amp;spell=1&amp;sa=X&amp;ei=_V6lUYejMKHl4APz9YBw&amp;ved=0CCsQvwUoAA&amp;bav=on.2,or.r_cp.r_qf.&amp;bvm=bv.47008514,d.dmg&amp;fp=e94ca269102e46e9&amp;biw=1416&amp;bih=728\" target=\"_blank\">Google News.<\/a><\/li>\n<\/ol>\n<p>And in response to queries from friends, family, and media, break out those three magic words doctors are often so reluctant to say &#8212; &#8220;I don&#8217;t know&#8221; or, if you want company, &#8220;We don&#8217;t know.&#8221;<\/p>\n<p>Because it&#8217;s the truth.<\/p>\n<p><em>(Also posted on <a href=\"http:\/\/commonhealth.wbur.org\/2013\/06\/worry-new-sars-like-virus\" target=\"_blank\">WBUR&#8217;s CommonHealth<\/a> site.)<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>From one of my close friends &#8212; a non-MD &#8212; comes this alarming video\u00a0(sorry, can&#8217;t remove the preceding ad).
