Interested to hear what you think about our new-ish look.<\/p>\n<p>In celebration, here are some quick ID\/HIV tidbits that have recently crossed my path, or have been sitting in my inbox for a while, dying to get out:<\/p>\n<ul>\n<li><a href=\"http:\/\/emergency.cdc.gov\/HAN\/han00349.asp\" target=\"_blank\"><strong>Doxycycline shortage<\/strong>.<\/a>\u00a0Hardly anything more frightening to a New England ID doc than a shortage of the world&#8217;s most versatile antibiotic <em>during peak Lyme season<\/em>. Shudder. Fortunately, most of my patients have had no trouble getting it from their pharmacies, though I do hear prices are way up.<\/li>\n<li>Some HCV news &#8212; USPSTF <strong><a href=\"http:\/\/annals.org\/article.aspx?articleid=1700383\" target=\"_blank\">recommends screening baby-boomers<\/a><\/strong>\u00a0(joining CDC, they had already recommended the same),\u00a0and an<strong>\u00a0<a href=\"http:\/\/www.fda.gov\/NewsEvents\/Newsroom\/PressAnnouncements\/ucm357982.htm\" target=\"_blank\">HCV genotype test is approved.<\/a><\/strong>\u00a0Here&#8217;s the question &#8212; why is the latter news at all? We&#8217;ve been sending HCV genotypes &#8212; &#8220;unapproved,&#8221; I guess &#8212; for years and years.<\/li>\n<li>More HCV: \u00a0Did you see the CDC study published this Spring showing that around <a href=\"http:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm6218a4.htm?s_cid=mm6218a4_w\" target=\"_blank\"><strong>half the\u00a0people with HCV don&#8217;t get a complete evaluation?<\/strong>\u00a0<\/a> Specifically, after a\u00a0positive antibody test, many have no subsequent HCV RNA (viral load) checked to determine whether they have chronic disease. How is that possible?<\/li>\n<li><strong><a href=\"http:\/\/www.medscape.com\/viewarticle\/805347?nlid=31747_801&amp;src=wnl_edit_medp_infd&amp;uac=36307HK&amp;spon=3\" target=\"_blank\">Another ID competition from John Bartlett.<\/a><\/strong> This time, it&#8217;s &#8220;Most Important Antiviral&#8221;. Go over to Medscape and vote and, more importantly, go read John&#8217;s fascinating discussion &#8212; which I bet he dictated between 4 and 5 a.m. early one day without using notes.<\/li>\n<li><strong><a href=\"http:\/\/journals.lww.com\/ccmjournal\/pages\/articleviewer.aspx?year=2013&amp;issue=07000&amp;article=00008&amp;type=abstract\" target=\"_blank\">A retrospective study says you can stop antibiotics\u00a0on patients with ventilator-associated pneumonia<\/a> <\/strong>if sputum cultures are negative. Yeah &#8212; <em>RIGHT<\/em>. (Readers of a certain age will say that with the full Bill Cosby effect.) NEJM Journal Watch summary <a href=\"http:\/\/www.jwatch.org\/na31360\/2013\/06\/11\/ventilator-associated-pneumonia-no-antibiotics-if-cultures\" target=\"_blank\">here<\/a>.<\/li>\n<li>Nice balanced piece in the\u00a0<em>New Yorker<\/em>\u00a0on<strong>\u00a0<a href=\"http:\/\/www.newyorker.com\/reporting\/2013\/07\/01\/130701fa_fact_specter?mbid=nl_Weekly%20(58)\" target=\"_blank\">the Lyme Disease controversy<\/a>.<\/strong> Kudos to the author for not implying that there are villains on <em>either<\/em> side &#8212; a common mistake when the issue is covered in the lay press.<\/li>\n<li>And on the subject of ticks, this time of the Lone Star variety, how about this bizarre connection &#8212; <strong><a href=\"http:\/\/online.wsj.com\/article\/SB10001424127887324634304578537203916053308.html\" target=\"_blank\">tick bites and allergy to red meat.<\/a>\u00a0<\/strong>Note that the allergic reaction is delayed, which may make the connection difficult to diagnose. The article includes the inevitable advice about wearing long pants and tucking them into your socks during the summer. Again &#8212; <em>RIGHT<\/em>. Perhaps no public health advice is less heeded than that one, beating out <strong><a href=\"http:\/\/www.nytimes.com\/video\/2013\/06\/24\/health\/100000002295116\/sex-for-science.html?smid=pl-share\" target=\"_blank\">even condom use.<\/a><\/strong>\u00a0(Love the interview at the start of that video, by the way &#8212; a young couple doing condom testing for &#8220;science&#8221;!)<\/li>\n<li>The Kaiser Family Foundation has released<strong> <a href=\"http:\/\/kaiserfamilyfoundation.files.wordpress.com\/2013\/06\/8449-mapping-the-donor.pdf\" target=\"_blank\">this fascinating report on the funding sources for global health related to HIV\/AIDS.<\/a><\/strong> Lots of interesting data, including the fact that the\u00a0U.S. provides almost two thirds of all HIV\/AIDS international assistance (61%), with the next largest donor, the Global Fund, providing one fifth &#8212; that&#8217;s around 80% of the total, if I&#8217;ve done my math right.<\/li>\n<li>Wouldn&#8217;t it be nice to have something that &#8220;eliminates up to 99.9% of airborne bacteria, mold, viruses&#8221;? And now, for just $299.95,<strong> <a href=\"http:\/\/www.hammacher.com\/Product\/81655?source=EMQ084&amp;cm_ven=WC&amp;cm_cat=20130618_EMQ084&amp;cm_pla=BYR&amp;cm_ite=81655_The%20Mold%20and%20Germ%20Destroying%20Air%20Purifier\" target=\"_blank\">it can be yours<\/a>.<\/strong> Good thing it isn&#8217;t $300.00, that would be <em>way<\/em> too expensive.<\/li>\n<\/ul>\n<p>By the way, wondering how to say &#8220;NEJM Group&#8221;? \u00a0It&#8217;s\u00a0<em>N-E-J-M Group<\/em>, spelled out, and not preceded by the word &#8220;The.&#8221; Now we all know.
