{"id":4957,"date":"2014-02-13T15:10:02","date_gmt":"2014-02-13T20:10:02","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=4957"},"modified":"2021-04-04T08:20:16","modified_gmt":"2021-04-04T12:20:16","slug":"jeter-is-retiring-and-certain-id-doctors-are-getting-older","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/jeter-is-retiring-and-certain-id-doctors-are-getting-older\/2014\/02\/13\/","title":{"rendered":"Jeter is Retiring, and Certain ID Doctors Are Getting Old(er)"},"content":{"rendered":"<p><a href=\"http:\/\/blogs.nejm.org\/wp-content\/uploads\/2014\/02\/derekjeter-rookie.jpeg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-4958\" src=\"http:\/\/blogs.nejm.org\/wp-content\/uploads\/2014\/02\/derekjeter-rookie.jpeg\" alt=\"derekjeter rookie\" width=\"239\" height=\"298\" srcset=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2014\/02\/derekjeter-rookie.jpeg 342w, https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2014\/02\/derekjeter-rookie-241x300.jpeg 241w\" sizes=\"auto, (max-width: 239px) 100vw, 239px\" \/><\/a>It&#8217;s safe to say that most of the perspectives on Derek Jeter&#8217;s retiring from baseball will not be written by ID doctors, so let me seize the opportunity.<\/p>\n<p>And since it&#8217;s always risky to dwell on players from <a href=\"http:\/\/www.urbandictionary.com\/define.php?term=MFY\" target=\"_blank\" rel=\"noopener\">a certain team<\/a> while living in Boston &#8212; I have friends for whom a central component of their identity is <a href=\"http:\/\/www.amazon.com\/Yankees-Suck-Official-Despise-Detest-ebook\/dp\/B002JF1NA0\/ref=sr_1_14?ie=UTF8&amp;qid=1392308806&amp;sr=8-14&amp;keywords=hate+yankees\" target=\"_blank\" rel=\"noopener\"><em>Hatred for the Yankees<\/em><\/a> &#8212; this will be less about Jeter&#8217;s retirement and more about how the news made me feel.<\/p>\n<p>In a word &#8212; old.<\/p>\n<p>You see, I was at the game when Jeter hit his first major league home run &#8212; <a href=\"http:\/\/www.baseball-reference.com\/boxes\/CLE\/CLE199604020.shtml\" target=\"_blank\" rel=\"noopener\">opening day in Cleveland, April 2 1996<\/a>, weather a balmy 38 degrees. That was a remarkable year on many fronts, both personal and professional, as my daughter was born, Boston experienced its <a href=\"http:\/\/www.erh.noaa.gov\/box\/climate\/snowbos.html\" target=\"_blank\" rel=\"noopener\">snowiest winter ever<\/a>, HIV became suddenly treatable (<a href=\"http:\/\/www.amazon.com\/How-About-That-Life-Allen\/dp\/B00CNKRPUS\/ref=sr_1_1?ie=UTF8&amp;qid=1392320962&amp;sr=8-1&amp;keywords=how+about+that+mel+allen\" target=\"_blank\" rel=\"noopener\">How About That!<\/a>), and well, the Yankees won the World Series, in part thanks to their rookie shortstop.<\/p>\n<p>So what was it like to be an ID doctor 18 years ago? Let&#8217;s take a look at the Wayback Machine, and I caution those of a certain age that this is guaranteed to make you feel downright ancient:<\/p>\n<ol>\n<li><span style=\"line-height: 13px\">You were still doing gram stains in a hospital lab that was on the patient floors.<\/span><\/li>\n<li>Aminoglycosides were a regular part of &#8220;triples&#8221; &#8212; you know, amp, <strong>gent<\/strong>, clinda.<\/li>\n<li>Amphotericin B had no liposomal formulation, and was co-administered with all kinds of magic potions (diphenhydramine, meperidine, hydrocortisone, pixie dust) to make it better tolerated.<\/li>\n<li>Erythromycin was used as an actual antibiotic &#8212; sometimes even intravenously.<\/li>\n<li>Ritonavir was used as an actual antiretroviral, and the dose was 600 mg twice daily (yes kids, you read that right).<\/li>\n<li>Severe <em>C diff<\/em> was extremely uncommon, and almost never happened in an outpatient.<\/li>\n<li>Community-acquired MRSA might be presented at an ID case conference given its rarity.<\/li>\n<li>There was hardly any vancomycin-resistant enterococcus &#8212; good thing, too, since there was no linezolid.<\/li>\n<li>If you wanted a quinolone to treat community-acquired pneumonia, you were out of luck &#8212; levofloxacin wasn&#8217;t approved until December 1996.<\/li>\n<li>If your patient needed indinavir, he\/she needed to get it from a single mail-order pharmacy called &#8220;Statscript.&#8221;<\/li>\n<li>No echinocandins, oseltamivir, tigecycline, cefepime, daptomycin.<\/li>\n<li>Some days you spent half of your time on rounds looking for X-rays. These were actual films, viewed on a light box &#8212; or, if you were in the radiology department, on those noisy contraptions that rotated light boxes. (What did they do with those things?)<\/li>\n<li>These numbers\/letters meant nothing: HLA-B*5701, HPTN 052, K65R, E138K,\u00a0CCR5, CXCR4, Q80K, PrEP, TDF.<\/li>\n<li>Target vancomycin trough level: \u00a05-10.<\/li>\n<li>Some people were actually taking ddC &#8212; which, despite the medical &#8220;information&#8221; from <em>Dallas Buyers Club<\/em>, was a pretty horrible medication.<\/li>\n<li>Best place for drug information was often the &#8220;PDR&#8221;. If you&#8217;ve never heard of it, don&#8217;t ask.<\/li>\n<\/ol>\n<p>I could go on like this all day, but why not finish on a high note?<\/p>\n","protected":false},"excerpt":{"rendered":"<p>It&#8217;s safe to say that most of the perspectives on Derek Jeter&#8217;s retiring from baseball will not be written by ID doctors, so let me seize the opportunity.
