OK, I&#8217;ve commented, and yes I&#8217;m hypersensitive]<\/em>\u00a0&#8212; one of my patients, a 38-year-old woman from Serbia, is about to start biologics\u00a0for rheumatoid arthritis <em>[you can see this is going to be a latent tuberculosis testing question from miles away].<\/em><br \/>\nShe&#8217;s not sure if she got BCG as a child <em>[yep, called that one]<\/em>, so I did\u00a0both <em>[uh-oh]<\/em> a PPD and sent a quantiferon gold <em>[note this is not the crazy alphabetizing\u00a0of the branded interferon gamma release assay (IGRA) products, which for some reason is &#8220;<span style=\"color: #545454\">QuantiFERON<\/span><span style=\"color: #545454\">\u00ae-TB\u00a0<\/span><span style=\"color: #545454\">Gold<\/span><span style=\"font-weight: bold;color: #545454\">&#8220;<\/span>\u00a0or the other one, which just goes with all-caps, &#8220;T-SPOT.TB&#8221;, and\u00a0throws a period in the middle &#8212; jeeze, marketers]<\/em>. The PPD is positive at 20 mm, but the quantiferon is negative <em>[of course &#8212; otherwise there&#8217;s no question].<\/em> Her CXR is negative.<br \/>\nMy conclusion is that she does not have TB and therefore does not need preventive therapy prior\u00a0to starting biologics <em>[that&#8217;s notable &#8212; putting your money down!]<\/em>. Is that the correct conclusion?<br \/>\nI&#8217;m sure this is a very easy question\u00a0<em>[ah, not so easy after all]<\/em> as you must get it all the time <em>[second part is most certainly true].<\/em><br \/>\nThank you very much sir <em>[so formal! or maybe he was in the military]<\/em> for your time.<br \/>\nArchie\u00a0<em>[not his real name, but\u00a0it was the name of a dog we had growing up, a long-haired dachshund]<\/em><\/p>\n<p>As noted above, nope, not so easy. Let&#8217;s consider the variables:<\/p>\n<ul>\n<li>She&#8217;s from a part of the world with a <a href=\"https:\/\/extranet.who.int\/sree\/Reports?op=Replet&amp;name=%2FWHO_HQ_Reports%2FG2%2FPROD%2FEXT%2FTBCountryProfile&amp;ISO2=RS&amp;LAN=EN&amp;outtype=html\" target=\"_blank\">higher rate of TB<\/a>\u00a0than we have here. So the prior probability of latent TB is greater than if she was US-born, though\u00a0it&#8217;s not as high <a href=\"https:\/\/extranet.who.int\/sree\/Reports?op=Replet&amp;name=%2FWHO_HQ_Reports%2FG2%2FPROD%2FEXT%2FTBCountryProfile&amp;ISO2=ZA&amp;LAN=EN&amp;outtype=html\" target=\"_blank\">as this.<\/a><\/li>\n<li>She&#8217;s about to get &#8220;biologics&#8221;, which by convention means\u00a0some sort of expensive immunosuppressive drug, probably a TNF-inhibitor. These are of course great potentiators of TB, some worse than others.<\/li>\n<li>She <em>may<\/em> have received BCG, which can give you a false-positive tuberculin skin test. But this effect typically wanes over time, and even if she did get BCG, it&#8217;s probably not very effective in preventing adult TB.<\/li>\n<li>The sensitivity of the skin test vs IGRA is all but impossible to determine &#8212; because there is <em>no gold standard for latent tuberculosis.<\/em><\/li>\n<\/ul>\n<p>To summarize, the positive skin test could be a true positive, or could be a false positive from the (maybe) BCG. And the negative IGRA could be a true negative, or could be a false negative because it&#8217;s not 100% sensitive.<\/p>\n<p>So what to do? Here was my answer:<\/p>\n<div style=\"color: #222222;padding-left: 30px\">Hi Archie,<br \/>\nThe problem is that there is no gold standard &#8212; either one could be right. You might consider sending a T-SPOT, since it <a href=\"http:\/\/annals.org\/article.aspx?articleid=742038\" target=\"_blank\">could be a bit more sensitive<\/a> than the QF. But since biologics are strong potentiators of active TB, in general I&#8217;d recommend preventive therapy for a couple of months before she starts it (then completing a full course) since she&#8217;s from a part of the world with more TB than we have here.<\/div>\n<div style=\"color: #222222;padding-left: 30px\">A formal consult would be reasonable if this isn&#8217;t clear, or if you\/she want.<\/div>\n<div style=\"color: #222222;padding-left: 30px\">Paul<br \/>\nPS: we had a dog named Archie when I was\u00a0a kid.<\/div>\n<p>So my question for you TB specialists out there &#8212; since this is such a common question, why doesn&#8217;t managing it appear in all the various guidelines and textbooks?
