{"id":646,"date":"2009-12-13T15:21:30","date_gmt":"2009-12-13T20:21:30","guid":{"rendered":"http:\/\/blogs.nejm.org\/?p=646"},"modified":"2009-12-13T15:21:30","modified_gmt":"2009-12-13T20:21:30","slug":"infection-and-the-icu-outcome-predictable-but-important","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/infection-and-the-icu-outcome-predictable-but-important\/2009\/12\/13\/","title":{"rendered":"Infection and the ICU:  Outcome Predictable, but Important"},"content":{"rendered":"<p>If you enrolled over 14,000 ICU patients into a study on a single day, and then did follow-up, what would you find regarding the relationship of infection to the outcomes of ICU stay and mortality?<\/p>\n<p>Just such a study was <a href=\"http:\/\/jama.ama-assn.org\/cgi\/content\/abstract\/302\/21\/2323\" target=\"_blank\">published in <\/a><em><a href=\"http:\/\/jama.ama-assn.org\/cgi\/content\/abstract\/302\/21\/2323\" target=\"_blank\">JAMA<\/a> <\/em>last week, and here are the not-so-stunning conclusions:<\/p>\n<blockquote><p>Infections are common in patients in contemporary<sup> <\/sup>ICUs, and risk of infection increases with duration of ICU stay.<sup> <\/sup>In this large cohort, infection was independently associated<sup> <\/sup>with an increased risk of hospital death.<\/p><\/blockquote>\n<p>To an ID specialist, this is kind of like reading that someone has done a study linking time spent outside in the rain and the likelihood of becoming wet.\u00a0 Patients in ICUs are susceptible to getting infections for innumerable reasons &#8212; so many that it seems to us (from our admittedly biased perspective) almost remarkable when an infection <em>doesn&#8217;t<\/em> occur.<\/p>\n<p>In all seriousness, ICU-related infections are a gargantuan problem, and if this study helps publicize the clinical and research needs, more power to it.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>If you enrolled over 14,000 ICU patients into a study on a single day, and then did follow-up, what would you find regarding the relationship of infection to the outcomes of ICU stay and mortality?
