Could it be that\u00a0the antibiotic-obesity association\u00a0will have a greater effect in reducing\u00a0outpatient use of antibiotics than the risk of resistance? <strong><em><a href=\"https:\/\/news.google.com\/news\/story?ncl=dXATyRsycFl50AMIDawEF3JPA_xYM&amp;q=antibiotics+obesity&amp;lr=English&amp;hl=en&amp;sa=X&amp;ved=0CCkQqgIwAGoVChMIwZjP5_fayAIVA3M-Ch3UAgcv\" target=\"_blank\">Lots<\/a><\/em><\/strong> of press coverage.<\/li>\n<li>At the other end of the age spectrum, here&#8217;s a thoughtful perspective on the widespread\u00a0<strong><a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=2451305\" target=\"_blank\">use of antibiotics at the end of life.<\/a><\/strong>\u00a0I&#8217;ve covered <strong><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/should-antibiotics-be-part-of-end-of-life-care\/2015\/02\/15\/\" target=\"_blank\">this topic before<\/a><\/strong> &#8212; why are antibiotics so often given at the end of life when all other medical interventions are stopped? Paper cites <strong><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20625013\" target=\"_blank\">one study<\/a><\/strong>\u00a0suggesting &#8220;greater comfort, albeit shorter survival, among patients with advanced dementia and suspected pneumonia who were not treated with antimicrobials.&#8221; Sounds like withholding antibiotics could be\u00a0the right move in palliative care.<\/li>\n<li>HCV therapy with <strong><a href=\"http:\/\/www.fda.gov\/Drugs\/DrugSafety\/ucm468634.htm\" target=\"_blank\">paritaprevir\/ombitasvir\/ritonavir may precipitate hepatic decompensation and\u00a0liver failure.<\/a>\u00a0<\/strong>Not surprisingly, &#8220;most&#8221; of the cases\u00a0(which have precipitated an FDA-mandated label change) have occurred in patients with advanced cirrhosis.<\/li>\n<li>There are infectious risks associated with receiving injections of live cells, including <strong><a href=\"http:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm6438a3.htm\" target=\"_blank\">Q fever from\u00a0sheep cells<\/a><\/strong> and <a href=\"http:\/\/ofid.oxfordjournals.org\/content\/2\/2\/ofv054.full?sid=cf27d0b7-af72-4e33-a5d8-a7add9b79141\" target=\"_blank\"><strong><em>M abscessus<\/em>\u00a0from human fetal cells.<\/strong><\/a>\u00a0Certainly infection is\u00a0one good reason not to undergo these treatments; the second and even better one is that none of these therapies has any proven medical benefit. A bit more on this longstanding form of quadkery <strong><a href=\"http:\/\/www.quackwatch.org\/01QuackeryRelatedTopics\/Cancer\/cellular.html\" target=\"_blank\">here.<\/a><\/strong><\/li>\n<li>The <strong><a href=\"http:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm6440a6.htm?s_cid=mm6440a6_w\" target=\"_blank\">ocular syphilis cluster\u00a0on the West Coast<\/a>\u00a0<\/strong>is a reminder that in the post-PrEP, post treatment-as-prevention world, condomless sex is inevitably increasing among MSM, along with rates of sexually transmitted infections. Note that this report repeats the recommendation that all patients with ocular syphilis should 1) undergo a CSF exam (which may be negative in ocular syphilis) and 2) be treated for neurosyphilis regardless of CSF results. Which prompts me to ask &#8212; then why do the CSF exam if treatment isn&#8217;t changed based on the results?<\/li>\n<li><strong><a href=\"https:\/\/idsa.confex.com\/idsa\/2015\/webprogram\/Paper53211.html\" target=\"_blank\">Transmitted\u00a0NNRTI resistance does not appear to influence response to integrase-based ART.<\/a><\/strong> This nice analysis presented at IDWeek from a Stanford\/Kaiser collaboration confirms previous reassuring findings from prospective clinical trials. And it means you probably don&#8217;t need to use a boosted PI in this situation.<\/li>\n<li>Promising results from a small (n=20) single-arm pilot study of <strong><a href=\"http:\/\/natap.org\/2015\/EACS\/EACS_04.htm\" target=\"_blank\">two-drug, dolutegravir + lamivudine ART in treatment-naive patients.<\/a><\/strong>\u00a0Think <strong><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/gardel-two-active-drug-study-not-a-game-changer-but-it-might-be-a-paradigm-shifter\/2013\/10\/25\/\" target=\"_blank\">GARDEL<\/a><\/strong> strategy, only with DTG and not LPV\/r. 20\/20 are virologically suppressed at week 24, and all but one were &lt; 50 by week 8. Note that the inclusion\u00a0criteria limited enrollment to patients with\u00a0HIV RNA &lt; 100,000 and CD4 &gt; 200. A larger (but still not comparative) clinical trial of this strategy is\u00a0<strong><a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02582684?term=5353&amp;rank=2\" target=\"_blank\">opening soon.<\/a><\/strong><\/li>\n<li>In the vaccine world, is there anything more complicated than the recommendations for meningococcal immunization? Based on a meeting that took place this June,\u00a0they&#8217;ve now changed again to <strong><a href=\"http:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/mm6441a3.htm?s_cid=mm6441a3_w\" target=\"_blank\">include the\u00a0serogroup B\u00a0vaccine<\/a>\u00a0<\/strong>for late adolescents.\u00a0When in doubt on vaccine-related issues, head over to the invaluable <strong><a href=\"http:\/\/www.immunize.org\/\" target=\"_blank\">immunize.org<\/a><\/strong>\u00a0site, in particular the &#8220;Ask the Experts&#8221; section.<\/li>\n<li><strong><a href=\"http:\/\/natap.org\/2015\/IDSA\/IDSA_21.htm\" target=\"_blank\">No clinically significant drug-drug interactions between ECF-TAF or RF-TAF and sofosbuvir\/ledipasvir.<\/a>\u00a0<\/strong>Even with the ledipasvir slightly boosting tenofovir exposure, levels\u00a0remain 5X lower than when TDF is used. ECF-TAF (&#8220;Stribild 2.0&#8221;) approval expected next month, RF-TAF (&#8220;Complera 2.0&#8221;) next year.<\/li>\n<li>Here&#8217;s a nifty case report: A 91-year-old man had\u00a0<em>10 separate hospital visits<\/em>\u00a0for fevers, chills, and hypotension over a two year period, several times\u00a0with\u00a0positive blood cultures for <em>Aeromonas hydrophila.\u00a0<\/em>The source?\u00a0<strong><a href=\"http:\/\/ofid.oxfordjournals.org\/content\/2\/4\/ofv142.full\" target=\"_blank\">Contaminated well water.<\/a><\/strong> My favorite quote from the paper: &#8220;Culture results revealed significant overgrowth of <em>A hydrophila<\/em> from his master bathroom sink; he used this water supply to soak his dentures nightly.&#8221;\u00a0Made me think of <strong><a href=\"https:\/\/youtu.be\/_39gkDAbMaI\" target=\"_blank\">this household product<\/a>\u00a0&#8212;\u00a0<\/strong>he&#8217;s old enough to have watched that ad.<\/li>\n<li>On the topic\u00a0of great cases, here&#8217;s a <strong><a href=\"http:\/\/blogs.nejm.org\/frontlines-clinical-medicine\/2015\/10\/20\/i-was-raised-on-a-zebra-farm\/\" target=\"_blank\">very nice inaugural piece on the In Practice blog<\/a><\/strong> by Harrison Reed, who reminds us that these rare and astounding cases &#8212; zebras &#8212; are vastly\u00a0outnumbered by the more common cause of hoofbeats, horses.<\/li>\n<\/ul>\n<p>Warning, adult content time.\u00a0In her HBO special, Amy Schumer not surprisingly has a plenty of\u00a0ID-related jokes &#8212; including this surprising one:<\/p>\n<blockquote><p>I was dating an Infectious Diseases doctor<em>, <\/em>cause\u00a0two birds &#8230;<\/p><\/blockquote>\n<p>How convenient!
