You Decide"},"content":{"rendered":"<p><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-7838\" src=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves-300x219.jpg\" alt=\"roll up sleeves\" width=\"300\" height=\"219\" srcset=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves-300x219.jpg 300w, https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves-1024x749.jpg 1024w, https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves-900x658.jpg 900w, https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2015\/11\/roll-up-sleeves.jpg 1172w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a>If you&#8217;re\u00a0not immersed in the ID or the Infection Control world, you might not be aware that there&#8217;s currently quite the controversy about whether doctors should wear white coats.<\/p>\n<p>I almost wrote &#8220;raging controversy&#8221; &#8212; but the adjective &#8220;raging&#8221; doesn&#8217;t\u00a0really fit the sort of people who specialize in Infection Control, who are\u00a0some of the most measured, data-driven, and methodical individuals\u00a0in all of medicine. You know the stereotype of the brash, volatile, and cowboy\u00a0surgeon, the person that everyone tiptoes around?<\/p>\n<p>These Infection Control folks\u00a0are the polar opposite.<\/p>\n<p>Still, a\u00a0white coat controversy does exist, and it\u00a0goes like this:<\/p>\n<ol>\n<li>You can culture all kinds of scary bacteria from\u00a0the sleeves of white coats.<\/li>\n<li>Doctors don&#8217;t launder their white coats\u00a0very often.<\/li>\n<li>These bacteria could be transmitted to patients.<\/li>\n<li>Therefore, doctors shouldn&#8217;t wear white coats.<\/li>\n<\/ol>\n<p>Not surprisingly, some have advocated\u00a0a &#8220;bare-below-the-elbows&#8221; approach to doctors&#8217; attire, and famously the British National Health Service has made it policy, banishing ties as well \u00a0(and predictably\u00a0<a href=\"http:\/\/www.bmj.com\/content\/346\/bmj.f3211\" target=\"_blank\" rel=\"noopener noreferrer\">not pleasing everyone<\/a>). There\u00a0was a <a href=\"http:\/\/www.hcplive.com\/conference-coverage\/idweek-2015\/should-doctors-go-bare-infection-control-debate-rages\" target=\"_blank\" rel=\"noopener noreferrer\">tremendously entertaining debate<\/a>\u00a0on the topic this year year at IDWeek &#8212; such a pitched battle it almost met the &#8220;raging&#8221; criteria, but not quite (you could tell the combatants really liked each other). Right here in Boston,\u00a0one of our current ID fellows <a href=\"https:\/\/theconversation.com\/its-time-for-doctors-to-hang-up-the-white-coats-for-good-47536\" target=\"_blank\" rel=\"noopener noreferrer\">feels quite passionately about the issue<\/a>, and writes frequently on the topic.<\/p>\n<p>The missing piece in the controversy, of course, is a\u00a0definitive study\u00a0showing that patient infection rates are actually reduced when a bare-below-the-elbows (often abbreviated &#8220;BBE&#8221;) strategy is adopted. Short of that, we&#8217;re all essentially\u00a0hostage to whomever sets these policies wherever we work. And our hospital&#8217;s policies basically follow those of\u00a0<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24442071\" target=\"_blank\" rel=\"noopener noreferrer\">Society for Healthcare Epidemiology of America (SHEA)<\/a>, which &#8220;balance professional appearance, comfort, and practicality with the potential role of apparel in the cross-transmission of pathogens.&#8221;<\/p>\n<p>In other words, wear white coats if you want, but keep them clean.<\/p>\n<p>This data void, however, gives me permission\u00a0to ruminate over some of the important and not-so-important questions that inevitably come up when contemplating white coats, and our\u00a0ambivalent feelings about\u00a0them.<\/p>\n<p>Specifically:<\/p>\n<ul>\n<li><strong>Why do some doctors wear them now, and some don&#8217;t?<\/strong> My wife the pediatrician said if\u00a0<strong><a href=\"http:\/\/hydeparkpedi.com\/about-us\/physicians\/\" target=\"_blank\" rel=\"noopener noreferrer\">she or one of her practice partners<\/a><\/strong> showed up in a white coat, the office staff would view it as bizarre as if they showed up dressed as a <strong><a href=\"https:\/\/www.google.com\/url?sa=i&amp;rct=j&amp;q=&amp;esrc=s&amp;source=images&amp;cd=&amp;cad=rja&amp;uact=8&amp;ved=0CAQQjBxqFQoTCP6tg-yB78gCFUc5JgodeBwGCw&amp;url=http%3A%2F%2Forig07.deviantart.net%2F790f%2Ff%2F2014%2F042%2F2%2Fc%2Froman_centurion_by_manzanedo-d7614ac.jpg&amp;bvm=bv.106379543,d.eWE&amp;psig=AFQjCNEKMY9ljyXT98fEaTTRFbVwVG3UhA&amp;ust=1446460077991433\" target=\"_blank\" rel=\"noopener noreferrer\">Roman centurion.<\/a><\/strong>\u00a0She didn&#8217;t actually say that, but you get the idea &#8212; pediatricians don&#8217;t avoid white coats because of infection risk, but because\u00a0they are\u00a0considered potentially scary to\u00a0kids. Meanwhile, the brilliant and fearsome\u00a0<strong><a href=\"http:\/\/www.amazon.com\/gp\/product\/B00ESK52D8\/ref=dp-kindle-redirect?ie=UTF8&amp;btkr=1\" target=\"_blank\" rel=\"noopener noreferrer\">Chief of Medicine<\/a><\/strong> during my residency considered us housestaff essentially naked unless we were wearing white coats, and\u00a0the Chief Medical Residents kept\u00a0a few extra\u00a0in their offices on days he took Residents&#8217; Report in case we showed up white coat-less\u00a0&#8212; kind of like\u00a0the maitre d&#8217; at the formal restaurant keeping\u00a0a few extra blazers in the coat\u00a0closet in case someone\u00a0arrives without a jacket.<\/li>\n<li><strong>What do the patients want?<\/strong>\u00a0Demonstrating that she was a far more compensated\u00a0medical student than I,\u00a0one of my classmates during medical school actually <strong><a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=363846\" target=\"_blank\" rel=\"noopener noreferrer\">published\u00a0a study evaluating patients&#8217; preferences on doctor attire<\/a>.\u00a0<\/strong>Yes, patients\u00a0liked white coats back then, jeans not so much. It&#8217;s one of <em>many<\/em>\u00a0such studies appearing in the medical literature\u00a0over the years; a systematic review\u00a0of them is summarized in this paper published in the <a href=\"http:\/\/bmjopen.bmj.com\/content\/5\/1\/e006578.long\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>British Medical Journal<\/strong>.<\/a> Consensus? Patients still seem to like them, but\u00a0of course for every opinion about the reassuring message of professionalism conveyed by the white coat, there&#8217;s an opposite view that they <strong><a href=\"http:\/\/haicontroversies.blogspot.com\/2015\/10\/white-coats-contribute-to-unsafe.html\" target=\"_blank\" rel=\"noopener noreferrer\">reinforce outdated hierarchical structures in healthcare.<\/a><\/strong><\/li>\n<li><strong>What should doctors wear if they don&#8217;t wear a white coat?<\/strong> The obvious first thought is scrubs, but there are a few problems here.\u00a0First, scrubs are basically\u00a0utilitarian pajamas, and can get pretty nasty unless washed daily. Second, they don&#8217;t offer much in the way of insulation, a huge issue in the\u00a0winter and in ubiquitous\u00a0over-air conditioned interiors. Third, scrubs\u00a0can be very unkind to certain body types, male and female alike. (You know what I mean.) Fourth, they have basically no storage space &#8212; cell phone, wallet, <em>that&#8217;s it &#8212;<\/em>\u00a0forget the reflex hammer, ophthalmoscope (<strong><a href=\"http:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/not-doing-the-retinal-exam-and-the-importance-of-acknowledging-limitations\/2015\/08\/30\/\" target=\"_blank\" rel=\"noopener noreferrer\">though it&#8217;s useless to me<\/a><\/strong>), or pocket manual, and wearing scrubs\u00a0makes your stethoscope a permanent half-necklace<em>.<\/em>\u00a0Finally, the public has lots of negative\u00a0feelings about scrubs &#8212; especially when they see doctors <strong><a href=\"http:\/\/www.wtsp.com\/story\/news\/investigations\/2014\/11\/18\/hospitals-lack-policies-on-scrub-use-in-public\/19244407\/\" target=\"_blank\" rel=\"noopener noreferrer\">wearing them outside the hospital,<\/a>\u00a0<\/strong>which means to adopt scrubs as standard gear means changing when you get to work, ugh.\u00a0And if not scrubs? Men probably clean their suits less often than they wash their white coats, and my daughter told me recently that button-up short sleeved shirts on men are a <strong><a href=\"http:\/\/www.examiner.com\/images\/blog\/replicate\/EXID3040\/images\/resized_dwight_schrute_the_office_short_sleeve_dress_shirt.jpg\" target=\"_blank\" rel=\"noopener noreferrer\">huge fashion faux-pas<\/a><\/strong>, to be avoided as much as backpacks with wheels\u00a0and belt clips for cell phones. Who knew?<\/li>\n<li><strong>If white coats go, what happens to\u00a0the\u00a0&#8220;White Coat Ceremony&#8221; during medical school?<\/strong>\u00a0Apparently, <strong><a href=\"http:\/\/humanism-in-medicine.org\/programs\/rituals\/white-coat-ceremony\/\" target=\"_blank\" rel=\"noopener noreferrer\">97% of medical schools<\/a><\/strong> now have a White Coat Ceremony, the purpose of which is\u00a0&#8220;to\u00a0welcome new students into the medical profession and to set clear expectations regarding their primary role as physicians by professing an oath.&#8221; Does that oath now include a specified\u00a0frequency of laundering, including minimum\u00a0water temperature and\u00a0the need to use bleach? By the way, you youngsters might be surprised to hear that these solemn and moving rituals are relatively new. Back when I was in medical school, they just left you in a room alone with a cadaver, turned out the lights, and timed how long you could stay there before you screamed for help.<\/li>\n<li><strong>How would <em>New Yorker<\/em> cartoons designate doctors if white coats were abandoned?<\/strong> This is a <em>very<\/em> important question. Take a <strong><a href=\"https:\/\/www.google.com\/search?site=&amp;tbm=isch&amp;source=hp&amp;biw=1387&amp;bih=783&amp;q=doctor+new+yorker+cartoons&amp;oq=doctor+new+yorker+cartoons&amp;gs_l=img.3..0i8i30.1563.5510.0.5675.26.24.0.2.2.0.135.2219.19j5.24.0....0...1ac.1.64.img..0.26.2226.GkCg6NeWLjE#tbm=isch&amp;q=new+yorker+doctor+cartoons\" target=\"_blank\" rel=\"noopener noreferrer\">look<\/a><\/strong> &#8212; the overwhelming\u00a0majority have the MD in a white coat, and those few that don&#8217;t typically involve surgeons in the OR. Here&#8217;s one of the\u00a0<strong><a href=\"http:\/\/imgc-cn.artprintimages.com\/images\/P-473-488-90\/60\/6067\/NHID100Z\/posters\/paul-noth-will-i-still-be-able-to-not-exercise-new-yorker-cartoon.jpg\" target=\"_blank\" rel=\"noopener noreferrer\">rare exceptions<\/a>\u00a0<\/strong>to the above rule\u00a0and 1) the guy is wearing long sleeves (not BBE!), and 2) it&#8217;s not <em>that<\/em> funny. I&#8217;d give it a 7 out of 10 on the funniness scale &#8212; now <strong><a href=\"http:\/\/33.media.tumblr.com\/c67179c87d58f52ddeb90e5a3bfd12d5\/tumblr_mga1b3oe6j1qav5oho1_500.gif\" target=\"_blank\" rel=\"noopener noreferrer\">this one<\/a><\/strong> is funny. Note that <strong><a href=\"https:\/\/www.google.com\/search?site=&amp;tbm=isch&amp;source=hp&amp;biw=1387&amp;bih=783&amp;q=leo+cullen+doctor+cartoons&amp;oq=leo+cullen+doctor+cartoons&amp;gs_l=img.3...1010.6666.0.6877.26.11.0.15.1.0.126.1028.9j2.11.0....0...1ac.1.64.img..15.11.947.vDoCXnF0q9I#tbm=isch&amp;q=leo+cullum+doctor+cartoons\" target=\"_blank\" rel=\"noopener noreferrer\">this brilliant cartoonist<\/a><\/strong> often adds the head mirror to the white coat, just so you couldn&#8217;t possibly think that the white-coated person in his cartoon might be an insurance salesperson or an airline pilot.<\/li>\n<\/ul>\n<p>Ok, so\u00a0controversy not resolved.
