Advances in HIV prevention mean they can drop their categorical\u00a0recommendation\u00a0against insemination with semen from HIV-infected men, one they originally made in 1990.<\/p>\n<p>But it&#8217;s strange because, right alongside treatment\u00a0of the HIV-positive man with antiretroviral therapy (ART) and pre-exposure prophylaxis (PrEP) for the woman, is\u00a0a fairly lengthy data summary on &#8220;sperm washing&#8221;, a strategy\u00a0many would argue has outlived its usefulness.<\/p>\n<p>This is the short version of the procedure:<\/p>\n<blockquote><p>Another strategy that can be used in conjunction with HAART and PrEP is collection and washing of the male partner\u2019s sperm to remove cells infected with HIV, followed by testing to confirm the absence of HIV prior to intrauterine insemination (IUI) of the female partner or in vitro fertilization (IVF).<\/p><\/blockquote>\n<p>Does one really need to put couples through this expensive and\u00a0time-consuming process when the risk of transmission is unmeasurably tiny, if not zero, if the man is on suppressive ART and the woman is taking PrEP? Can you imagine the <a href=\"http:\/\/www.thennt.com\/thennt-explained\/\" target=\"_blank\" rel=\"noopener noreferrer\">number needed to treat<\/a> to prevent one additional case of HIV transmission with sperm washing in addition to ART and PrEP?<\/p>\n<p>It&#8217;s like wearing a belt, suspenders, <em>and<\/em> using duct tape to keep your pants up (which is probably not the best analogy when discussing something that has to do with sex, but I&#8217;m going\u00a0with it).<\/p>\n<p>As a quick reminder:<\/p>\n<ul>\n<li><a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa1600693#t=article\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>HPTN 052:<\/strong> <\/a>\u00a0Zero transmissions from infected partner if HIV RNA suppressed.<\/li>\n<li><a href=\"http:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2533066\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>PARTNERS Study:<\/strong><\/a> \u00a0Zero transmissions from infected partner if HIV RNA suppressed &#8212; this while the couples were practicing &#8220;condomless sex&#8221; (otherwise they couldn&#8217;t be part of the study).<\/li>\n<li><strong>PrEP Studies:<\/strong> \u00a0Among adherent participants, &gt; 90% efficacy despite high risk behavior or high community risk.<\/li>\n<\/ul>\n<p>Now we don&#8217;t want to tempt the\u00a0Gods of Never Say Zero with hubris about this unmeasurably low risk,\u00a0but I clearly wasn&#8217;t the only one who found the paper&#8217;s emphasis on sperm washing peculiar.<\/p>\n<p>Here&#8217;s a take from Ben Young,\u00a0Senior Vice President\/Chief Medical Officer of the <a href=\"http:\/\/www.iapac.org\/AboutUs01.html\" target=\"_blank\" rel=\"noopener noreferrer\">International Association of Providers of AIDS Care<\/a>:<\/p>\n<p>https:\/\/twitter.com\/benyoungmd\/status\/870675504004648960<\/p>\n<p>(&#8220;U=U&#8221; stands for &#8220;undetectable equals uninfectious&#8221;.)<\/p>\n<p>Just for fun, I thought I&#8217;d check in with Pietro Vernazza, architect of the 2008\u00a0controversial <a href=\"http:\/\/www.aidsmap.com\/Swiss-experts-say-individuals-with-undetectable-viral-load-and-no-STI-cannot-transmit-HIV-during-sex\/page\/1429357\/\" target=\"_blank\" rel=\"noopener noreferrer\">&#8220;Swiss Statement&#8221;<\/a> that has, for the record, <a href=\"http:\/\/www.thebody.com\/content\/78550\/hiv-undetectable-does-equal-uninfectious-the-swiss.html\" target=\"_blank\" rel=\"noopener noreferrer\">turned out to be 100% correct.<\/a><\/p>\n<p>Here was his response:<\/p>\n<blockquote><p>I was shocked about the report. In Switzerland, we have not seen any new infections among couples trying to conceive without using any additional safeguards (aside from treatment of the infected partner). The Swiss statement is very widely accepted.<\/p><\/blockquote>\n<p>And then, because what the hell, we deserve it, he added this:<\/p>\n<blockquote><p>But what should we say? We also don&#8217;t understand how a country can withdraw from\u00a0the Paris agreement, or try to build a wall to Mexico, or to withdraw health insurance for their citizens.<\/p><\/blockquote>\n<p>Hey, I&#8217;m a huge fan of our CDC, this odd report notwithstanding.
