Phew.<\/p>\n<p>But what an interesting case! The requester was, to their credit, extremely polite and grateful, which is always appreciated.<\/p>\n<p><strong><em>Second:<\/em><\/strong>\u00a0 Another clinician sent me a curbside that was completely the opposite &#8212; something relatively concise, and straightforward, but alas I couldn&#8217;t be of much help given the unusual patient profile.<\/p>\n<p>Doubt there will be a HIPAA violation here if I share:<\/p>\n<p><a href=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2018\/01\/dog-twitter.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-8804\" src=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2018\/01\/dog-twitter.jpg\" alt=\"\" width=\"570\" height=\"310\" srcset=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2018\/01\/dog-twitter.jpg 570w, https:\/\/blogs.nejm.org\/hiv-id-observations\/wp-content\/uploads\/sites\/2\/2018\/01\/dog-twitter-300x163.jpg 300w\" sizes=\"auto, (max-width: 570px) 100vw, 570px\" \/><\/a><\/p>\n<p><strong><em>Third:<\/em><\/strong>\u00a0 Medscape recently reviewed the <a href=\"https:\/\/www.medscape.com\/viewarticle\/883538_1\" target=\"_blank\" rel=\"noopener\">medicolegal aspects of curbside consults<\/a>, an area of significant concern among ID doctors who lead the world in this activity. It&#8217;s an excellent, authoritative piece, though unfortunately the net effect could be to make one terrified of providing informal advice to our colleagues &#8212; something quite useful both to them and their patients.<\/p>\n<p>Take a look at this definition of a legally safe curbside consult:<\/p>\n<blockquote><p>A curbside consultation refers to a consultation where the patient isn&#8217;t identified; where the specialty physician typically doesn&#8217;t review the chart, doesn&#8217;t meet with the patient, doesn&#8217;t talk to the patient, and doesn&#8217;t examine the patient; where the specialty physician doesn&#8217;t have any direct or contractual agreement with the treating physician or the hospital where the treating physician is working; where the specialist physician doesn&#8217;t bill for his services; and where the treating physician is free to accept or reject the specialty physician&#8217;s advice.<\/p><\/blockquote>\n<p>Yikes, those are a lot of criteria! Oh well.<\/p>\n<p><strong><em>Fourth:<\/em><\/strong> <a href=\"http:\/\/thecurbsiders.com\/about-us\/meet-the-curbsiders\" target=\"_blank\" rel=\"noopener\">These general internists<\/a> do a whole podcast of curbsides, on multiple medical topics &#8212; so much so that they call themselves <em>The Curbsiders!<\/em>\u00a0No actual patients are discussed, of course, just imaginary ones (and no golden retrievers either).<\/p>\n<p>It&#8217;s a terrific show for people in both hospital- and outpatient-based practice, and they invited me on recently to do one on <a href=\"http:\/\/thecurbsiders.com\/internal-medicine-podcast\/78-id-pearls-paul-sax-md\" target=\"_blank\" rel=\"noopener\"><em>ID Pearls<\/em><\/a>. We covered cellulitis, Lyme, <em>C. diff<\/em>, flu vaccine, travel medicine, antibiotic allergies &#8212; in short, the very stuff we ID docs are curbsided about <em>all the time.<\/em><\/p>\n<p>Highly recommended.<\/p>\n<blockquote class=\"twitter-tweet\">\n<p dir=\"ltr\" lang=\"en\">Have you listened to our latest episode?
