{"id":93,"date":"2008-10-15T20:26:40","date_gmt":"2008-10-16T01:26:40","guid":{"rendered":"http:\/\/blogs.nejm.org\/pov\/hiv-id-observations\/?p=93"},"modified":"2008-10-15T20:26:40","modified_gmt":"2008-10-16T01:26:40","slug":"back-to-school-day-1","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/back-to-school-day-1\/2008\/10\/15\/","title":{"rendered":"Back to School, Day 1"},"content":{"rendered":"<p>We offer two post-graduate courses each year, one entitled <a href=\"http:\/\/cme.hms.harvard.edu\/index.asp?SECTION=CLASSES&amp;ID=00281220&amp;SO=N\" target=\"_blank\"><em>Infectious Diseases in Primary Care<\/em><\/a>, and the other<em> <a href=\"http:\/\/cme.hms.harvard.edu\/index.asp?SECTION=CLASSES&amp;ID=00281578&amp;SO=N\" target=\"_blank\">AIDS Medicine:\u00a0 An Intensive Case-Based Course<\/a><\/em>.\u00a0 The Primary Care one started today, the AIDS course starts on Monday.<\/p>\n<p>(Both are equally fascinating.\u00a0 I am entirely unbiased.)<\/p>\n<p>What is so striking is that the participants &#8212; and content &#8212; barely overlap at all.\u00a0 Topics for primary care course:<\/p>\n<ul>\n<li>Antibiotics<\/li>\n<li>UTIs<\/li>\n<li>Pneumonia<\/li>\n<li>Sore throats\/colds<\/li>\n<li>Immunizations<\/li>\n<\/ul>\n<p>You get the idea.\u00a0 For the AIDS course?<\/p>\n<ul>\n<li>Acute HIV infection<\/li>\n<li>Management of treatment-naive patients<\/li>\n<li>Interpretation of resistance testing<\/li>\n<li>Metabolic complications of therapy<\/li>\n<li>Legal and ethical considerations of HIV care<\/li>\n<\/ul>\n<p>I suppose I should not find the lack of overlap surprising, given <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18248723?ordinalpos=3&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_DefaultReportPanel.Pubmed_RVDocSum\" target=\"_blank\">papers such as this one<\/a>, citing that 62% of family practitioners refer their AIDS patients to specialists immediately &#8212; a big change from 1994, when only 18% did.\u00a0 (Even 62% seems low &#8230;)<\/p>\n<p>But the irony is that with improved antiretroviral therapy &#8212; the very thing that drove some generalists away from HIV care due to its complexity &#8212; HIV patients are living longer, and hence are in greater need of the kind of care delivered best by primary care clinicians.<\/p>\n<p>So maybe what we <em>really<\/em> need is a primary care course for HIV specialists?<\/p>\n","protected":false},"excerpt":{"rendered":"<p>We offer two post-graduate courses each year, one entitled Infectious Diseases in Primary Care, and the other AIDS Medicine:\u00a0 An Intensive Case-Based Course.\u00a0 The Primary Care one started today, the AIDS course starts on Monday.
