{"id":9738,"date":"2020-07-26T16:14:10","date_gmt":"2020-07-26T20:14:10","guid":{"rendered":"https:\/\/blogs.nejm.org\/hiv-id-observations\/?p=9738"},"modified":"2020-08-20T14:04:30","modified_gmt":"2020-08-20T18:04:30","slug":"time-to-amplify-our-voices-calling-for-inexpensive-rapid-home-testing-for-covid-19","status":"publish","type":"post","link":"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/time-to-amplify-our-voices-calling-for-inexpensive-rapid-home-testing-for-covid-19\/2020\/07\/26\/","title":{"rendered":"Time to Amplify Our Voices Calling for Inexpensive Rapid Home Testing for COVID-19"},"content":{"rendered":"<p>Earlier this month, I highlighted how <a href=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/rapid-inexpensive-home-testing-for-covid-19-may-get-us-out-of-this-mess-before-a-vaccine\/2020\/07\/05\/\" target=\"_blank\" rel=\"noopener noreferrer\">inexpensive rapid home testing for COVID-19 could get us out of this mess faster than a vaccine.<\/a><\/p>\n<p>To spare you re-reading the whole thing, here are the main points.<\/p>\n<p>Imagine a simple test done on a saliva sample placed on a paper strip. Results back in 15 minutes. Available without a doctor&#8217;s order. A couple of dollars per test. Would be as easy to perform as a home pregnancy test.<\/p>\n<p>And, most importantly, the results will highly correlate with what we care about the most when testing a person without symptoms &#8212; <em>are they infectious to others?<\/em><\/p>\n<p>With testing applied broadly, and a negative test in hand, think of the implications!<\/p>\n<p>Outpatient visits and procedures. Hospital visitors. Schools and universities. Air travel. Public transportation. Fitness clubs and gyms. Educational, business, and research conferences. Hair salons and barbershops. Bars and restaurants. Houses of worship. Nursing homes. Meat-packing plants, agricultural jobs, transit, and other high-risk work. Choral practices. Cruises. Homeless shelters. Dormitories and military barracks. Spectator sports. Prisons and jails.<\/p>\n<p>Endless, really.<\/p>\n<p>Prototypes for these tests, or slightly more complex versions, <a href=\"https:\/\/www.nature.com\/articles\/d41586-020-02140-8\" target=\"_blank\" rel=\"noopener noreferrer\">already exist.<\/a> So what&#8217;s the hold-up?<\/p>\n<p>Even though numerous companies and academic groups are working on such tests, there&#8217;s no guarantee the FDA will approve them &#8212; especially since they&#8217;re going to be less sensitive than the gold standard PCR tests we use for symptomatic people.<\/p>\n<p>But this lack of sensitivity shouldn&#8217;t block availability, since the tests likely <em>will<\/em> be positive during the few days that people have the highest titers of virus in their respiratory tract, and hence are most contagious. The quantity of virus in this early period (as measured by cycle thresholds) often is millions-fold (or more) higher than just a week later. Rapid tests done regularly should be able to capture this period of exponential viral growth.<\/p>\n<p>And a reasonably accurate test with results back quickly is far better than no test at all &#8212; or, as is sometimes happening, a test done with <a href=\"https:\/\/www.nytimes.com\/2020\/07\/24\/opinion\/covid-test-turnaround.html\" target=\"_blank\" rel=\"noopener noreferrer\">frustrating and potentially dangerous delays in results due to testing backlogs.<\/a><\/p>\n<blockquote class=\"twitter-tweet\" data-width=\"500\" data-dnt=\"true\">\n<p lang=\"en\" dir=\"ltr\">Hey <a href=\"https:\/\/twitter.com\/michaelmina_lab?ref_src=twsrc%5Etfw\">@michaelmina_lab<\/a>, this one&#39;s for you! h\/t <a href=\"https:\/\/twitter.com\/boulware_dr?ref_src=twsrc%5Etfw\">@boulware_dr<\/a> <a href=\"https:\/\/twitter.com\/StarTribune?ref_src=twsrc%5Etfw\">@StarTribune<\/a> SteveSack. <a href=\"https:\/\/t.co\/waA3PpKtAO\">pic.twitter.com\/waA3PpKtAO<\/a><\/p>\n<p>&mdash; Paul Sax (@PaulSaxMD) <a href=\"https:\/\/twitter.com\/PaulSaxMD\/status\/1286823867315556366?ref_src=twsrc%5Etfw\">July 25, 2020<\/a><\/p><\/blockquote>\n<p><script async src=\"https:\/\/platform.twitter.com\/widgets.js\" charset=\"utf-8\"><\/script><\/p>\n<p>At this point I should note the major inspiration on this topic all along has been my brilliant colleague, Dr. Michael Mina, who has become <a href=\"https:\/\/www.nytimes.com\/2020\/07\/03\/opinion\/coronavirus-tests.html\" target=\"_blank\" rel=\"noopener noreferrer\">a persuasive and passionate champion of the rapid home testing strategy.<\/a> Here he and economics professor Laurence J. Kotlikoff advocate approving these tests and then making them available to everyone:<\/p>\n<blockquote><p>Once paper strips\u2019 efficacy is definitively proved and they are cleared by the F.D.A., Congress can quickly authorize the production and distribution, for free, of a year\u2019s supply to all Americans. Then we\u2019ll have not only a true day-to-day sense of Covid-19\u2019s path. We\u2019ll also have a far better means to quickly contain and end this terrible plague.<\/p><\/blockquote>\n<p>Michael has been making the rounds pushing the idea, and fortunately, it&#8217;s starting to get a broader audience. I mention it often enough that my immediate family must wonder whether I&#8217;ve replaced my baseball obsession with a new one. (They may be right.)<\/p>\n<p>And while I read with interest <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMsr2022263?query=featured_home\" target=\"_blank\" rel=\"noopener noreferrer\">about how the NIH plans to rapidly scale up testing<\/a>, the home testing component of the program gets scant mention.<\/p>\n<p>Full disclosure, I have my own personal reasons for wanting easily available rapid tests &#8212; reasons that go beyond wanting to go out to a nice dinner, to start up my neighborhood poker game, or to visit Fenway for some baseball. The cultural activities of the city that so enriched their lives &#8212; movies, plays, operas, lectures, book clubs &#8212; have vanished. Even worse, they&#8217;ve had minimal non-Zoom contact with family or friends, including their children, grandchildren, or great-grandchildren.<\/p>\n<p>Their life now isn&#8217;t great. But they grew up during the depression, lived through World War II, and as a result have maintained a cheerful perspective all along that takes into consideration how things <em>could be so much worse.<\/em> One of their friends, for example, had COVID-19 and is still recovering, months later. And they know that hundreds of thousands have died.<\/p>\n<p>But now, my father needs major surgery. The hospital strictly limits visitors &#8212; one a day. And so the barrier to human contact &#8212; on our being able to visit and to comfort him safely &#8212; becomes more than just an inconvenience. It&#8217;s frankly heartbreaking.<\/p>\n<p>So send good thoughts his way. He&#8217;s <a href=\"https:\/\/blogs.nejm.org\/hiv-id-observations\/index.php\/on-fathers-day-a-tribute-to-a-father-who-isnt-allowed-to-celebrate-fathers-day\/2019\/06\/16\/\" target=\"_blank\" rel=\"noopener noreferrer\">quite something,<\/a> as I&#8217;ve mentioned.<\/p>\n<p>Finally, take 17 minutes and watch this excellent video on the science and rationale behind rapid home testing &#8212; it draws heavily on a longer interview Michael did on <a href=\"https:\/\/youtu.be\/kDj4Zyq3yOA?t=140\" target=\"_blank\" rel=\"noopener noreferrer\">This Week in Virology<\/a>.<\/p>\n<p>And if you have friends in powerful places, let&#8217;s push to make this potential breakthrough a reality &#8212; if not as national policy, can we do it at the state or city level?
