    Thought of your Z-Pak Part 1 on New Years Eve when the ER doc started me on IV azithromycin plus another, less familiar IV antibiotic for bilateral pneumonia (responsible organism unknown). Fortunately, they must have worked since I went home two days later somewhat better. I appreciate all your insights.

    In India Azithromycin along with Meropenem and ceftriaxone are the drugs which are still working for MDR enteric fever sir
    Even quinolones have lost their efficasy

      Absolutely. Typhoid fever is a great existing indication for azithromycin — you are correct!

    Is azithromycin still the best option for acute traveler’s diarrhea?

      Yes, we do recommend it for this indication — if treatment is recommended at all. Treatment recommendations for travelers’ diarrhea are stratified by symptom severity, with mild cases often requiring only supportive care (bismuth, loperamide, hydration) while moderate to severe cases benefit from antibiotic therapy.

    I seem to recall azithromycin being touted for it’s anti-inflammatory effects when treating COPD/chronic bronchitis exacerbations, even if its antibacterial effect was muted/absent.

    Or am I having yet another brain fart?

      Yes, you are correct. We ID doctors don’t generally like use of azithromycin for this “off target” effect since it selects for more resistance, prolongs QT, etc.
